New Security System Improves Client Safety and Compliance
Mitchell Wallick Ph.D. CAP Executive Director of C.A.R.E. today announced the activation of a new state of the art security system. Cameras at strategic points on C.A.R.E. property allow mental health technicians and supervisory staff to monitor all strategic areas in the drug rehab center at all times. This means that clients are under even better observation and safer than ever before.
Dr Wallick indicated that unlike many so called “Florida Model” programs C.A.R.E. has always maintained 24/7 awake supervision of clients. Unfortunately with two staff, only two areas of the building could be observed at a time. With the institution of the new system, staff can literally view all ten strategic areas. This allows them to better track clients and assist them if they are needed. Furthermore, supervisory staff, including clinical and executive director is able to observe even if they are off grounds. In the event of a medical issue, the nurse can view the client remotely and help in the decision beyond just the staff description. Said Wallick, “this is a fantastic piece of equipment. With it, you can be two places at the same time.”

A WORD TO PARENTS AND FAMILIES

I am sick! Maybe as a therapist I am not supposed to speak that way, but I am sick at heart. I am disgusted, angry, and just plain upset. Today I had watched one of my patients being killed by his family. I can not tell you the exact date of his death, but their cowardice signed his death warrant.
Using typical terrorist tactics, this patient threatened, cajoled and bullied his family into sending him a ticket home prior to the completion of addiction treatment. He is going home to his apartment, his old friends and old habits with the promise, “I will not use again”. This was his third time in rehab. This, his parents told him once again, is the last time that they were going to fund treatment. The fact that they had told him that time and time again is immaterial. They are still expecting to do the same thing and receive different results. The definition of this behavior is “insanity”.
Let me be blunt. When a child is little and steps into the street we spank his bottom. We do this not because we want to hurt the child and/or that we are brutal people. We do it because we as parents recognize that the natural consequences of the child’s behavior will be far worse than the spank on the bottom. The same applies to forcing a person to stay in treatment, regardless of their threats and pleading. It is far easier (spelled selfish) to give into demands than to give the person what they need, even if they don’t want it.
If these comments offend you, I apologize. On the other hand I would hope that you would prefer to have the person angry at you for a few days than be angry at yourself for the rest of your life.

Dr Mitchell Wallick, Executive Director of C.A.R.E. will be presenting two workshops at the 7th annual conference of addictive disorders on June 27th , in Fort Myers Florida. Dr Wallick’s first workshop will be on the importance of Humor in Counseling. This workshop will demonstrate interactive techniques of incorporating humor into both counseling and life. The workshop is designed to help prevent “terminal seriousness”. Dr Wallick will also be presenting on the topic of “Contracting for Behavior”.
The keynote speakera at the conference will be the Leo Booth and William Janes Director, Florida Office of Drug Control and Assistant Secretary For Mental Health and Substance Abuse. In Medicine there is no such diagnosis as addiction. What are commonly referred to as addiction are really two diagnosis categories. They are: Abuse and Dependency. When we speak of abuse, we are talking about using a drug to feel better than OK. When we speak of dependency we are talking about using a drug just to feel OK. We can further define dependency as both physiological and psychological. In physical dependency, as the name implies, the patient has physical symptoms of withdrawal. Drugs which cause physical dependency are alcohol and other Central Nervous system depressants. Withdrawal from these drugs without proper medical treatment can result in seizures and death. It is essential that persons addicted to these drugs be treated in a proper medical facility. The other classes of drugs which require medical detoxification are opiates. In the case of opiates, the patient withdrawing will look and feel like they are having the very worst case of the flu that can be imagined. They are going to feel like they are going to die, and are sorry when you tell them they are not. Medical detox and/or other medications can be used to make the patient feel more comfortable during the physical withdrawal process.
It is important to understand that all that a medical detox addresses is the physical symptoms involved with the withdrawal process. It in no way addresses the underlying needs of the patient that causes the problem. This is the reason why we see again and again, patients telling us I have detoxed any number of times and it just does not work. It is for this reason that treatment is needed.
In treatment, at a program like C.A.R.E., the patient learns to deal with their problems. Instead of self medicating, they learn to deal with the stressors and alternative ways of dealing with pain and discomfort whether it be physical and/or mental. Those with emotional problems are stabilized on medications and seen by a psychiatrist. Mental health counselors and groups are involved in providing a better way of life.

After 40 + years of practice, I have reached the conclusion that addiction is usually a syndrome rather than a single disease. That is not to say that addiction is not a disease, but rather to say that it often appears in combination with other things.
Let us consider for a few moments the underlying causes of many addictions. We have found that addiction is generally related to people underlying issues. It is difficult to tell if the drug addiction has caused these issues, or these issues have been caused by the addiction, but non-the-less the bottom line is that addiction often appears in combination with mental and physical illnesses. When one or more illnesses appear together, we call this phenomenon a dual diagnosis.
Let’s think about how this works. The second or third diagnosis causes pain, emotional discomfort, or feelings that are not comfortable. A sure way to get rid of them is to self medicate. Unfortunately, most of our patients have not attended medical school. This means that generally speaking they will choose the wrong medication, more often than not with disastrous results.
In seeking treatment, one should choose a program which will do at least the following:
Identify underlying problems. A full evaluation which includes gambling, eating disorders, physical and mental illnesses etc; is essential in pinpointing what must be treated.
Personnel and/or available consultants should be prepared to deal with all of the problems.
A good program like C.A.R.E. will educate the patient about switching addictions and the complications that, that can cause.

This is being written in response to a frequently asked question. We hear again and again on the phone, how I can possibly be an addict if I don’t use chemicals and/or alcohol.
The first part of the answer is that we do not really have a diagnosis of addiction in the DSM IV which is what clinicians use to help diagnose patients. Instead we use two terms. The first is abuse, the second is dependency. The simplest, but also in my opinion the most relevant definitions of these words are as follows:
Abuse—the use of a substance to feel better than ok.
Dependency—the use of a substance just to feel ok.
Since we do need to talk about addiction, even though it is not a medical diagnosis, I am going to use the following working definition. An addiction is acting in a way that we know is not good for us…but feels that we must keep doing it anyway. When we gamble, participate in sexual acts, steal, lie etc. without good reason, but still feel compelled to do so, we are acting as if we are addicted. We know the results are going to be bad…but we do it anyway.
Process addictions are same as chemical dependency in that the person suffers a compulsion to behave in an unhealthy way. This can often include healthy activities such as exercise or eating that are being done to excess. This means that the healthy activity is hurting us because we are doing too much of it.
Process addictions often hide underlying problems that the individual just does not wish to face. For example gambling is often used as a mechanism to escape from unpleasant feelings. Sex is often used to punish others and/or overcome feelings of inferiority. Finally eating is a form of convincing a person that there is something they can control. Each of these of course has an unhealthy outcome and must be dealt with to prevent the individual from continuing to harm themselves and return to a quality life.

Patients at different stages of their illness require different levels of service.
Intensive Outpatient Program:
Often individuals who are ready to transition and/or have a strong home environment are able to enter either our Outpatient or Intensive Outpatient Treatment Program.
Our Intensive Outpatient Program provides a minimum of ten hours of group and individual counseling with sessions from three evenings per week to six full days per week in treatment.

There are many triggers to relapse for recovering alcoholics and addicts, but there are also preventive measures that can be successful.
C.A.R.E. has a relapse prevention program designed specifically for our alumni. These two programs can at times be very appropriate in assisting with addiction recovery. Education for Chemical Dependency is also offered at C.A.R.E.. This program is very beneficial in helping residents learn more about their specific addiction, as well as in encouraging them to take their motivation for recovery to the next level.
For those who choose to return home, care will continue to provide telephone support, alumni weekends and be available to help you with any problems that occur during transition and beyond. In order to ensure continued progress, patients leaving care receive a continuing care plan which will include:
An appointment with a local therapist who will receive (with the patient’s permission) C.A.R.E. Records.
A list of available meetings and resources.
A schedule of activities for the first few weeks of transition.
Continuing telephone communication with their C.A.R.E. primary therapist and other counselors

Dual Diagnosis Drug treatment Facilities

Multiple illnesses can often co-exist in a single person. Coexisting Disorders are commonly called dual diagnosis. Conservative estimates state that sixty to seventy percent of people entering treatment has one or more co-existing conditions. Frequently, these multiple conditions complicate recovery. Patients overcome one area then fall victim to another condition in a vicious cycle of relapse and frustration.
Co-existing Conditions and Dual Diagnosis at C.A.R.E.
At C.A.R.E. we believe in treating the whole individual. We know that pain, emotional and/or physical can contribute to drug and alcohol use. We are fully aware that addiction can further complicate mental and physical ailments. Many treatment centers do not specialize in dual diagnosis or co-existing disorders but our counselors and treatment program specialists recognize the existance of a co-existing disorder and define an entire treatment program around dual diagnosis patients attacking the mental disorder itself as well as the additction.
C.A.R.E.'s multi-disciplinary team will help you address each of these areas as our specialists work with you to identify the underlying causes of your problem and address them.
Intensive Dual Diagnosis Program
Our dual diagnosis program is comprehensive. Patients in the program are assigned a primary therapist who is a Masters level individual specializing in Addiction Treatment and Mental Health. A minimum of one weekly Psychiatric treatment team visit is scheduled with the multi-disciplinary team which consists of the Psychiatrist, Clinical Director, Primary Counselor, Nurse, Coordinator of Alternative Medicine and other appropriate individuals as required by the patient’s individualized treatment plan. When needed our community resource specialist, who is a licensed clinical social worker, will provide continuing case management. Those requiring supervised living and continuing care can join our continuing care program after graduation.

Intensive 14 Day Compulsive Gambling Treatment
|
Intensive treatment program combines a fourteen day module of a highly intensive residential treatment experience with planned aftercare in your home area and planned weekend campus experiences.
Should it be determined that you are eligible for this fast track program, you will have to be prepared to put in full and intensely busy days. Your day will begin at 8:30 AM and you can expect to be involved in therapeutic activities till 10:00 AM every evening. In addition to meeting with the Psychiatrist individually, you may expect a minimum of one individual session per day with either your primary and/or gambling counselor. What causes drug addiction?
Not everyone will become addicted to the drugs they take, but do certain things impact our susceptibility to them? Drug addiction can be affected by the following:
Genetics affect our susceptibility to drugs. Different drugs affect our body system in different ways, for instance, one person may enjoy the effects of a drug while another may not due to chemicals and signals affecting the brain. Drugs in the category of Cannabinoids contain delta-9-tetrahydrocannibinol (THC), which sends out signals throughout the body. For some, the results are pleasing, but for others, the results are not. Also, someone who is directly related to an addict has a higher chance of becoming one than another person might.
Another important aspect of drug addiction is its availability. Also, each drug has its own physical makeup and thus each drug varies in the time it takes to become addicted. Past trauma is a factor that causes many people to use drugs. Sexual abuse, loss of a loved one or some sort of an accident are all things someone would want to forget. Often times, people will deal with these problems and temporarily allow themselves to forget about them by using drugs.
Another factor to be considered is the influence of peers in one’s life. Different drugs affect our body system in different ways, for instance, one person may enjoy the effects of a drug while another may not due to chemicals and signals affecting the brain. Drugs in the category of Cannabinoids contain delta-9-tetrahydrocannibinol (THC), which sends out signals throughout the body. For some, the results are pleasing, but for others, the results are not. Too many times in our society, that question goes unanswered and the people who need help the most don’t get it.
